학습 자료 둘러보기 →The priority intervention for a client with hyphema is to elevate the head of the bed to 30-45 degrees and enforce strict bed rest. This uses gravity to promote inferior settling of blood in the anterior chamber, clearing the visual axis and facilitating drainage through the trabecular meshwork to lower intraocular pressure (IOP).
The primary goal is to prevent a secondary hemorrhage (rebleed), which typically occurs 3 to 5 days post-injury. A rebleed is often more severe and carries a high risk of permanent vision loss from corneal blood staining and uncontrolled glaucoma. Maintaining the head-elevated, supine position stabilizes the fragile initial clot.
Avoid any activities that can increase IOP or disrupt the clot, such as eye movements, reading, bending, or straining. Do not administer antiplatelet or anticoagulant medications. Protecting the eye with a shield is essential, but positioning and rest take immediate priority.
