Protect the eye with a metal shield, not a pressure patch. Instruct the patient to avoid Valsalva maneuver, bending, lifting, and sudden movements. Administer prescribed topical corticosteroids and cycloplegics, but avoid mydriatics which can stretch torn vessels.
Monitor for rebleeding, which peaks at 3-5 days post-injury. Any sudden increase in pain or IOP is an emergency. Do not administer aspirin or NSAIDs due to antiplatelet effects.
