Although modern evidence shows the allergen in shellfish is tropomyosin (not iodine), and true iodine allergy is not physiologically possible, a history of severe reactions to any substance requires careful evaluation. The risk is for an anaphylactoid reaction to the dye, which is related to its osmolarity, not iodine content.
Mildly elevated blood glucose (180 mg/dL) and blood pressure (150/90 mmHg) are not contraindications but require documentation. Mild eye discomfort is an expected symptom of the underlying diabetic retinopathy.
Do not dismiss a documented "iodine allergy" without investigation. The priority is reporting the history so the provider can assess risk, consider premedication, or choose an alternative diagnostic approach. Emergency equipment must be available during dye injection.
