Sustained high intravesical pressure transmits backward, causing obstructive uropathy. This can lead to hydronephrosis and acute kidney injury. A critical risk is forniceal rupture, where urine extravasates into the perirenal space.
Prioritize immediate bladder decompression via catheterization. Gross hematuria, pain, and bruising are expected findings; however, the absence of urine output in a patient with a palpable bladder or urge is the most time-sensitive threat to renal function.
