학습 자료 둘러보기 →In nephrotic syndrome, massive proteinuria leads to hypoalbuminemia and decreased oncotic pressure, causing fluid to shift into interstitial tissues. Despite visible edema, the intravascular volume is often depleted, activating the RAAS to retain sodium and water.
The priority intervention is daily weight and strict intake and output (I/O) monitoring. A change of 1 kg in body weight equates to 1 liter of fluid retention or loss, making it the most sensitive, non-invasive indicator of fluid balance.
Do not encourage increased fluids or administer a high-sodium diet. These actions worsen fluid overload. Instead, implement a low-sodium diet and possible fluid restriction as prescribed, while maintaining adequate protein intake to compensate for losses.
