학습 자료 둘러보기 →A serum creatinine increase of ≥0.3 mg/dL within 48 hours or to ≥1.5 times baseline within 7 days confirms AKI per KDIGO guidelines. For a patient with a baseline of 1.0 mg/dL, a rise to 1.5 mg/dL meets both criteria.
Always compare current creatinine to the patient's known baseline, not just the laboratory reference range. A value within "normal" limits can still represent a significant decline in GFR for that individual.
Do not rely solely on BUN or edema for early AKI diagnosis. BUN is influenced by non-renal factors, and edema is a late and nonspecific sign. Oliguria is a key criterion but may not be the earliest indicator in all patients.
