학습 자료 둘러보기 →In a client with acute COPD exacerbation showing anxiety and restlessness, the first action is to position them in High Fowler's. This independent intervention immediately optimizes ventilation-perfusion (V/Q) matching by maximizing diaphragmatic excursion and chest expansion without any risk of harm.
An ABG showing pH 7.28, PaCO2 70 mmHg, and HCO3- 30 mEq/L indicates a partially compensated respiratory acidosis. The elevated PaCO2 confirms CO2 retention, making the client dependent on the hypoxic drive. Aggressively increasing oxygen risks blunting this drive and worsening respiratory failure.
Do not administer anxiolytics or sedatives as a first-line intervention. These can depress the respiratory center and mask worsening hypoxia. Anxiety in this context is often a symptom of air hunger and will improve as breathing mechanics are optimized.
