A change in breathing pattern, such as irregular respirations with periods of apnea, signals direct brainstem compression and is an immediate threat to life.
Other critical signs like Cushing's triad (hypertension, bradycardia, and irregular respirations) and pupillary changes (unequal, sluggish) indicate late-stage ICP and impending herniation. While these require urgent reporting, they are addressed only after the airway and breathing are secured.
A GCS drop of 2 points or more is a significant neurological emergency. However, do not let it distract you from a simultaneous airway or breathing crisis. Prepare for immediate airway management and notify the provider while continuing to assess.
