The pain radiates along the affected nerve's dermatome, most commonly the sciatic nerve (L4-S3), traveling from the lower back down the posterior leg to the foot.
This differs from nociceptive pain, which is typically dull, aching, and localized to the lower back without radiation, often seen in muscle strain.
Bilateral leg pain or cramping that worsens with walking suggests neurogenic claudication from spinal stenosis, not a unilateral disc herniation. A thorough neurological exam is essential.
