Understanding the Priority: Neurovascular Assessment in Acute Fracture Care
When a client sustains a closed fracture of a long bone such as the tibia, the immediate hours following the injury represent a critical window for monitoring. While the fracture itself involves a disruption in bone continuity, the primary threat to the limb in the acute phase is not the bone injury alone, but the potential for secondary damage to the surrounding soft tissues, nerves, and blood vessels. The priority nursing intervention is to
assess the neurovascular status of the affected extremity.
The Pathophysiological Basis for Neurovascular Compromise
The rationale for this priority is rooted in the pathophysiology of acute compartment syndrome and direct neurovascular injury. Tibial plateau fractures, particularly those resulting from high-energy trauma, are not simple breaks; they are complex injuries with significant associated soft-tissue damage

Why Other Interventions Are Not the First Priority

Applying ice and elevation: These are interventions aimed at reducing edema and pain. Paresthesia: Ask the client about any numbness, tingling, or "pins and needles" sensation. This is often an early and reliable sign of nerve ischemia.
