The classic triad includes a low serum iron, a low serum ferritin (typically <30 ng/mL), and a high TIBC. Red blood cell morphology will show microcytosis and hypochromia.
Serum ferritin is an acute-phase reactant. A normal or elevated ferritin does not rule out iron deficiency in patients with chronic inflammation, infection, or liver disease. In these cases, TIBC is a more reliable marker of true iron status.
