The obstruction causes a buildup of conjugated bilirubin in the bloodstream. The kidneys excrete this excess bilirubin, darkening the urine, while the absence of bilirubin reaching the gut results in acholic, pale stools.
Prolonged obstruction can rapidly progress to life-threatening cholangitis and coagulopathy from vitamin K malabsorption. Assess for Courvoisier's sign (a palpable, non-tender gallbladder) and anticipate urgent decompression via ERCP or PTC.
