Clinical Context
This scenario describes a laboring primigravida at term with strong, frequent contractions and advanced cervical dilation (8 cm). The fetal heart rate (FHR) tracing reveals a baseline of 110-120 bpm with moderate variability, but the presence of late decelerations with each contraction is the critical finding. A late deceleration is a visually apparent, gradual decrease in the FHR that begins after the peak of the contraction and returns to baseline after the contraction ends. The nadir of the deceleration occurs after the peak of the contraction. This pattern is a hallmark of uteroplacental insufficiency, where the fetus is not receiving adequate oxygenation during the stress of uterine contractions [1,3].
The immediate priority is to improve fetal oxygenation and maximize uteroplacental perfusion. The correct nursing action is to reposition the client to the left side and administer oxygen. The left lateral position displaces the gravid uterus off the maternal aorta and inferior vena cava, thereby increasing venous return, maternal cardiac output, and ultimately uterine blood flow. Administering oxygen at 8-10 L/min via a non-rebreather face mask aims to increase the maternal partial pressure of oxygen, creating a larger diffusion gradient to enhance oxygen transfer to the fetus across the compromised placenta. These are standard, first-line intrauterine resuscitation measures. The presence of moderate variability is a reassuring sign that the fetal central nervous system is still intact and responsive, which supports attempting these conservative interventions before proceeding to emergent delivery.
Analysis of Incorrect Options
- Option 2: Prepare for immediate cesarean delivery and notify the anesthesiologist. While persistent late decelerations that do not resolve with intrauterine resuscitation measures would indicate a need for expedited delivery, this is not the immediate priority action. The cervix is already 8 cm dilated, and the fetal baseline variability is moderate, suggesting the fetus is not yet severely acidotic. The nurse must first implement interventions to correct the presumed hypoxia. If the pattern persists despite position change, oxygen, and IV fluid bolus, then notification of the provider for possible operative delivery becomes the next step [1,3].
- Option 3: Increase the IV fluid rate and encourage the client to push with contractions. Increasing the IV fluid rate is an appropriate intrauterine resuscitation measure to improve maternal blood volume and uterine perfusion, but it is not the priority action over position change and oxygen administration. Encouraging the client to push is contraindicated. The cervix is fully effaced but only 8 cm dilated, meaning it is not yet completely dilated (10 cm). Pushing before complete dilation can cause cervical edema and laceration, and the repeated Valsalva maneuver can further compromise fetal oxygenation by reducing maternal cardiac output and worsening the existing late decelerations.
- Option 4: Document the findings and continue to monitor for 15 more minutes. Documentation is an essential nursing function, but it is not the priority action when a nonreassuring FHR pattern is identified. Late decelerations are a sign of fetal compromise requiring immediate intervention, not passive observation. Delaying action for 15 minutes could allow the fetus to progress from hypoxia to metabolic acidosis, increasing the risk of neonatal morbidity and mortality [2,3].
Pathophysiology and Clinical Reasoning
The underlying mechanism of late decelerations is a reflex response to transient fetal hypoxemia during a uterine contraction. In a labor with adequate placental reserve, the intermittent reduction in blood flow during a contraction is well-tolerated. However, when placental function is compromised—as can occur in post-term pregnancies (41 weeks) due to placental aging and calcification—the contraction-induced decrease in oxygen delivery causes fetal myocardial depression and a subsequent drop in heart rate after the contraction’s peak. This is detected by peripheral chemoreceptors and baroreceptors, triggering a vagal response that slows the heart rate [1,3]. The systematic review by Yilak et al. confirms that nonreassuring fetal heart rate status is a significant predictor of adverse immediate neonatal outcomes, emphasizing the need for prompt recognition and intervention .
The presence of moderate variability is a crucial distinguishing factor. Variability represents the interplay between the sympathetic and parasympathetic nervous systems and is a direct indicator of fetal cerebral oxygenation. Moderate variability (amplitude range 6-25 bpm) is a strong predictor of a normal fetal acid-base status, even in the presence of decelerations. This finding gives the nurse a therapeutic window to perform intrauterine resuscitation. If the variability were absent or minimal, the pattern would be more ominous, suggesting metabolic acidosis and a more urgent need for immediate delivery . The immediate outcomes of nonreassuring patterns, as studied in the Ethiopian context, highlight that timely interventions during the intrapartum period are critical to reducing preventable perinatal morbidity [1,2].
