Understanding the Clinical Context
Pulmonary tuberculosis during pregnancy presents a significant diagnostic and management challenge. The physiological adaptations of pregnancy, including a relative suppression of cell-mediated immunity, can mask classic symptoms and alter disease progression, potentially delaying diagnosis and increasing the risk of adverse outcomes for both the mother and fetus
[1]. When assessing a pregnant client with a known TB diagnosis, the nurse must prioritize findings that indicate active, progressive, or complicated disease, which pose the greatest immediate threat.
Analysis of Assessment Findings
To determine the most concerning finding, each option must be evaluated against the typical clinical picture of TB in pregnancy and the potential for maternal-fetal harm.

Fetal heart rate of 110 beats per minute (Option 3): A fetal heart rate of
110 bpm is at the lower limit of the normal baseline range, which is 110-160 bpm. By itself, this is a reassuring finding indicating a living fetus without acute distress. While maternal TB can lead to adverse perinatal outcomes like preterm birth or intrauterine growth restriction, a single heart rate of 110 bpm is not a direct sign of an acute maternal complication

Productive cough with blood-tinged sputum (Option 4): This is the most concerning finding. Hemoptysis, or blood-tinged sputum, indicates erosion of a bronchial artery or cavitary lesion into the pulmonary vasculature. This finding signifies active, destructive lung pathology. It is a marker of high bacillary load and increased transmission risk, but more critically, it is a direct warning sign for a potentially life-threatening massive hemoptysis. This acute event can rapidly compromise maternal airway, breathing, and circulation, requiring immediate medical intervention. The physiological changes of pregnancy, such as increased vascularity, may theoretically exacerbate bleeding, making this an urgent priority.
Clinical Reasoning and Priority Setting
Using the nursing process and a "first things first" approach focused on airway, breathing, and circulation (ABCs), a productive cough with blood-tinged sputum (hemoptysis) represents an immediate threat to the mother's airway and respiratory status. The other options—low-grade fever, gradual weight loss, and a borderline-normal fetal heart rate—are all clinically significant findings associated with TB in pregnancy that require ongoing monitoring and management. However, they do not signal an impending, acute, life-threatening event in the same way that hemoptysis does. The nurse's most concerning assessment is the one that heralds a potential for rapid clinical deterioration, making option 4 the correct priority.
