Understanding the Procedure and Immediate Risks
Orchiopexy is a surgical procedure to mobilize a testicle, bring it down into the scrotum, and fix it in place. In a 2-year-old with bilateral cryptorchidism, this involves manipulation within the inguinal canal and scrotum. The immediate postoperative period is critical because the highly vascular scrotal and inguinal tissues are prone to edema and hematoma formation. The primary goal of nursing assessment is to detect any compromise to the testicular blood supply or the formation of a scrotal hematoma, which could lead to increased pressure, pain, and potential necrosis of the surgically placed testicle. The research by Tao et al. underscores that the testicular salvage rate in children with cryptorchidism who experience a complication like torsion is critically time-dependent, with a salvage rate of only
18.2% when symptom duration exceeds 24 hours, compared to
100% when addressed within 6 hours
[1]. While this study focuses on torsion, the underlying principle of maintaining vascular integrity is directly transferable to the post-orchiopexy state, where surgical edema or a concealed bleed can similarly compromise microvascular flow to the testicle.
Why Monitoring the Surgical Site is the Priority
The most important intervention is to
monitor the surgical site for signs of bleeding or swelling. This is not merely a wound check; it is a focused assessment for a developing scrotal hematoma or excessive edema that can create a compartment-like syndrome within the scrotum. An expanding hematoma increases pressure on the spermatic cord structures, potentially compressing the delicate testicular artery and pampiniform plexus of veins. This pressure can lead to venous congestion, arterial insufficiency, and ultimately, testicular ischemia. The clinical study highlights that in cryptorchid patients, the anatomical abnormalities of the gubernaculum and processus vaginalis predispose them to such vascular events
[1]. Therefore, a nurse’s vigilant, serial assessment—documenting scrotal size, color, and tenderness—is the frontline defense for preserving the viability of the testicle. Detecting a firm, tense, bluish scrotal mass early allows for prompt surgical re-exploration, which is the definitive intervention to prevent testicular loss.
Why Other Options are Less Appropriate

Encouraging early ambulation in a 2-year-old is inappropriate. The standard of care for scrotal surgery is to minimize physical activity and provide scrotal support to prevent traction on the spermatic cord, reduce pain, and limit the risk of postoperative bleeding from the surgical site.

Applying ice packs continuously is contraindicated without a protective barrier and frequent skin assessments, especially in a young child with delicate skin. While cold therapy can reduce edema, continuous application risks vasoconstriction that could paradoxically impair blood flow to the newly positioned testicle, a risk that is unacceptable given the critical nature of testicular perfusion.

Positioning the child in a prone position would place direct pressure on the tender surgical site, increasing pain and potentially disrupting the suture line. The appropriate comfort measure is to position the child supine or side-lying with a rolled towel or diaper for scrotal elevation to promote venous return and reduce swelling.
