Clinical Priority in RSV Bronchiolitis with Hypoxemia
The infant presents with classic signs of moderate-to-severe respiratory distress secondary to RSV bronchiolitis: tachypnea with a respiratory rate of
70/min, grunting, and subcostal retractions. The critical finding that drives the priority intervention is the oxygen saturation of
88% on room air, which confirms
hypoxemia. In the hierarchy of nursing care for pediatric respiratory distress, correcting hypoxemia is the immediate physiological priority. Hypoxemia can rapidly lead to bradycardia and respiratory arrest in infants, who have a high metabolic rate and limited cardiopulmonary reserve. Positioning the infant in a
semi-Fowler's position optimizes diaphragmatic excursion and lung expansion, while administering
supplemental oxygen directly addresses the life-threatening oxygen deficit. This intervention aligns with the foundational airway-breathing-circulation (ABC) framework, where ensuring adequate oxygenation is a core component of managing "Breathing."
While bronchodilator therapy and chest physiotherapy may be considered in the broader management plan, they are not the immediate priority. The evidence for routine bronchodilator use in viral bronchiolitis is weak, and current guidelines do not recommend it as standard care. Chest physiotherapy has not demonstrated consistent benefit in reducing disease severity or length of stay in RSV bronchiolitis and can potentially increase distress in an already compromised infant. Encouraging oral fluids is contraindicated in a patient with severe tachypnea and grunting due to the high risk of aspiration; these infants often require intravenous or nasogastric fluid support to maintain hydration while protecting the airway.
