The most concerning assessment finding requiring immediate nursing intervention is
decreased appetite and irritability during feeding attempts. While the other options represent expected or progressive signs of respiratory distress, this finding is a subtle but critical indicator of a potentially life-threatening complication: severe
hyponatremia secondary to the
syndrome of inappropriate antidiuretic hormone secretion (SIADH).
Pathophysiology: Why This Finding Signals Danger
In infants with RSV bronchiolitis, the pulmonary inflammation and hypoxia can trigger a non-osmotic release of antidiuretic hormone (ADH) from the posterior pituitary gland, leading to SIADH [1,2]. ADH acts on the renal collecting ducts to increase water reabsorption, resulting in dilutional hyponatremia. As the serum sodium level drops, the osmotic gradient shifts, causing water to move into brain cells, leading to cerebral edema. The earliest neurological manifestations of this process in a pre-verbal infant are often nonspecific and easily overlooked:
irritability and a sudden
decrease in appetite or refusal to feed

Clinical Application and Nursing Priority
The nurse must recognize that an infant with RSV is at risk for SIADH. A change in neurological status, even as subtle as new irritability and feeding refusal, is a red flag for developing hyponatremia and possible cerebral edema

The immediate nursing intervention is to report this finding promptly to the healthcare provider, anticipate orders for serum electrolyte levels (specifically sodium), and implement strict intake and output monitoring, as fluid restriction is a cornerstone of SIADH management . This prioritization follows the principle of addressing the most life-threatening complication first: neurological compromise from electrolyte imbalance takes precedence over managing expected respiratory symptoms.
