Hepatitis A virus (HAV) infection in the pediatric population typically presents with a self-limiting course, but the clinical spectrum can range from asymptomatic infection to
fulminant hepatitis, a life-threatening complication
[1]. When assessing a 12-year-old with confirmed hepatitis A, the nurse must prioritize findings that suggest a progression from simple hepatic inflammation to acute liver failure.
The most concerning finding is
dark amber urine and decreased urine volume. While dark urine is a common finding in hepatitis due to the renal excretion of excess
conjugated bilirubin, the combination of dark urine with
oliguria (decreased urine volume) signals a dangerous escalation. This presentation is highly suggestive of
hepatorenal syndrome or severe dehydration secondary to the systemic inflammatory response, both of which are hallmarks of fulminant hepatitis. In the context of HAV, the progression to fulminant hepatic failure, though rare, is the primary cause of mortality and requires immediate intervention
[1]. Decreased urine output indicates that the kidneys are no longer being adequately perfused, a sign of hemodynamic instability that can rapidly progress to multi-organ failure.
Analysis of Other Options
The remaining findings are expected clinical manifestations of uncomplicated acute viral hepatitis and do not, in isolation, indicate a need for immediate life-saving intervention.
