Understanding the Priority: Recognizing Acute Liver Failure
The most significant finding to report immediately is an
altered level of consciousness and confusion. In a child with hepatitis A, this neurological change is the hallmark of
hepatic encephalopathy, which signals a progression from uncomplicated acute hepatitis to
acute liver failure (ALF). ALF is a life-threatening emergency defined by the onset of severe liver injury with
coagulopathy and hepatic encephalopathy within
26 weeks of symptom onset in a patient without pre-existing chronic liver disease

1% of hepatitis A cases progress to ALF, the condition carries a high mortality rate, with survival ranging from
30-60% without transplantation
[2]. Deterioration can be abrupt and unpredictable, making early recognition and immediate reporting critical for timely referral to a transplant center

Why the Other Options Are Less Immediately Critical
The other findings represent the expected clinical picture of acute viral hepatitis and do not, by themselves, indicate a life-threatening shift in the disease trajectory.

cholestasis (impaired bile flow) due to liver inflammation. They reflect the underlying pathology but do not signal the catastrophic loss of liver function seen in ALF.

Nausea, vomiting, and decreased appetite: These are common prodromal symptoms of acute hepatitis A infection, as the virus is typically transmitted via the fecal-oral route and causes initial gastrointestinal upset . They are expected and require supportive care, not emergency intervention.

Jaundice of the sclera and skin: This is a cardinal sign of hepatitis resulting from the liver's inability to process bilirubin. Its sudden onset is characteristic of the icteric phase of the illness . While important to document, jaundice itself is not the immediate threat that encephalopathy represents.
Clinical Reasoning and Pathophysiology
The transition from hepatitis to ALF is a critical juncture. The liver's synthetic and detoxification functions fail, leading to the accumulation of neurotoxins like ammonia, which cross the blood-brain barrier and cause cerebral edema and the neurological symptoms of hepatic encephalopathy. The initial presentation of hepatitis A often includes non-specific symptoms like nausea and anorexia, followed by jaundice and dark urine . The nurse must differentiate these expected manifestations from the ominous sign of a changing mental status. A previously alert child who becomes confused or lethargic is demonstrating a failure of the liver's metabolic function, a development that overrides all other clinical signs in urgency. This is because ALF can progress rapidly, and outcomes are heavily dependent on prompt intensive care and evaluation for liver transplantation
