A 4-year-old child is in the preoperational stage of cognitive development. They interpret words literally and fear the unknown. Avoid abstract terms like "cut" or "put to sleep" which can be terrifying.
The priority intervention is to use simple, concrete language and provide therapeutic play. Allow the child to handle safe medical equipment (e.g., a mask, bandages) on a doll to familiarize them with the experience and express their feelings.
Never dismiss a child's emotions by telling them to "be brave." Validate their separation anxiety and fear. Avoiding the topic completely increases distress because the imagined scenario is often worse than the reality.
