Understanding the Priority: Airway and Safety First
For a client with amyotrophic lateral sclerosis (ALS) experiencing progressive muscle weakness and dysphagia, the highest priority nursing intervention is to implement aspiration precautions and modify diet consistency. This decision is rooted in the fundamental nursing principle of the ABCs (Airway, Breathing, Circulation). Progressive bulbar involvement in ALS leads to dysfunction of the swallowing mechanism, directly threatening airway patency.
Pathophysiology of Dysphagia in ALS
The dysphagia in ALS is a form of
neurogenic dysphagia (ND) resulting from the degeneration of both upper and lower motor neurons that control the swallowing muscles. The central pattern generator (CPG) for swallowing, located in the medulla oblongata, can be disrupted by the disease process, leading to a loss of coordinated swallowing

Clinical Implementation of the Priority Intervention
The nurse’s immediate actions must focus on preventing aspiration. This begins with a clinical swallowing evaluation and implementing compensatory strategies. Modifying diet consistency based on a speech-language pathologist's (SLP) assessment is a cornerstone of management. This typically involves thickening liquids to a nectar or honey-like consistency to slow their transit, allowing the client more time to coordinate swallowing and airway closure . Aspiration precautions also include positioning the client upright at a 90-degree angle during meals, ensuring a distraction-free environment, and having suction equipment readily available. The goal is to maintain safe oral intake for as long as possible, as severe dysphagia significantly reduces quality of life and can lead to the need for discussions about alternative nutrition or, in extreme cases, interventions like aspiration prevention surgery .
