Clinical Reasoning and Priority Setting
The most concerning finding is
decreased breath sounds in the lower lung fields with shallow respirations. This assessment indicates
hypoventilation and impending
respiratory failure, which is the leading cause of death in
amyotrophic lateral sclerosis (ALS). In ALS, progressive degeneration of both upper and lower motor neurons leads to weakness of the diaphragm and intercostal muscles. When breath sounds diminish in the bases and the respiratory pattern becomes shallow, it signals that the patient can no longer maintain adequate tidal volume or effective gas exchange. This is a life-threatening emergency requiring immediate intervention, such as alerting the provider, preparing for noninvasive ventilation support, and obtaining arterial blood gas analysis.
Why the Other Options Are Lower Priority
Muscle fasciculations in the arms and legs (Option 1) are a hallmark sign of lower motor neuron involvement in ALS. While distressing to observe, fasciculations are an expected manifestation of the disease process and do not represent an acute threat to airway, breathing, or circulation. They require monitoring and documentation but not emergency intervention.
Difficulty with fine motor tasks such as buttoning clothes (Option 2) reflects progressive upper extremity weakness and loss of motor function. This finding is consistent with disease progression and warrants occupational therapy referral and adaptive equipment planning. However, it is a chronic functional decline, not an immediate physiological crisis.
Emotional lability with episodes of inappropriate crying (Option 3) describes
pseudobulbar affect (PBA), a common symptom in ALS resulting from corticobulbar tract degeneration. While this symptom can significantly impact quality of life and requires pharmacological management, it does not pose an imminent risk to the patient’s survival. Respiratory status always takes precedence in the nursing prioritization framework of airway, breathing, and circulation.
Pathophysiology and Clinical Significance of Respiratory Compromise in ALS
The respiratory system is critically vulnerable in ALS due to progressive denervation of the respiratory muscles. As the phrenic nerve and intercostal nerves lose their motor neuron input, the diaphragm and intercostal muscles atrophy and weaken. This leads to a restrictive ventilatory defect characterized by reduced vital capacity and impaired cough effectiveness. The finding of decreased breath sounds in the lower lung fields with shallow respirations is a late and ominous sign. It suggests that the tidal volume has fallen to a point where air entry is barely perceptible on auscultation, and the patient is at extreme risk for
hypercapnic respiratory failure, atelectasis, and secretion retention leading to pneumonia.

Any additional insult — whether pharmacological, metabolic, or infectious — can precipitate rapid decompensation. Therefore, a nurse's detection of shallow respirations and diminished breath sounds must trigger an immediate escalation of care to prevent respiratory arrest
