Understanding the Priority Complication After Total Knee Replacement
Following a total knee replacement (TKA), the most serious and life-threatening complication the nurse must prioritize is a
venous thromboembolism (VTE), which includes both
deep vein thrombosis (DVT) and
pulmonary embolism (PE). While pain, anxiety, and swelling are important and require intervention, they do not pose the same immediate risk of mortality as a PE. The foundational principle of postoperative TKA care is balancing clot prevention with the risk of bleeding, a significant clinical burden highlighted in the literature
[3]. The nurse's priority action is to implement measures that directly prevent venous stasis, a key factor in
Virchow's triad for thrombus formation.
Why Early Ambulation and Exercise is the Priority
Encouraging early ambulation and range of motion exercises directly counteracts venous stasis by activating the calf muscle pump, which promotes venous return from the lower extremities. This mechanical method of prophylaxis is a cornerstone of care and works synergistically with pharmacological agents. The critical importance of preventing VTE is underscored by research identifying multiple independent risk factors for PE after TKA, including advanced age, elevated BMI, and prolonged operative time
[2]. A patient who is immobile due to pain and anxiety is at a significantly heightened risk. By prioritizing mobility, the nurse is actively mitigating the risk of a potentially fatal PE, which is the most serious complication. While anticoagulants like
rivaroxaban or
low molecular weight heparin (LMWH) are prescribed for pharmacological prophylaxis, the nurse’s independent and immediate intervention to promote venous return is to facilitate safe movement .
Analysis of Other Options
The other options are important components of care but address secondary concerns rather than the priority life-threatening complication.

Administering analgesics (Option 2): This is a crucial intervention to facilitate participation in physical therapy, as pain is a significant barrier to mobility. However, it is an enabling step for the priority intervention, not the priority itself. The goal of pain control is to allow for early ambulation.

Applying ice packs (Option 3): This intervention primarily addresses localized swelling and provides mild analgesia through vasoconstriction. It does not systemically prevent DVT or PE.

Elevating the affected leg (Option 4): Elevation aids in reducing edema by using gravity to promote venous and lymphatic drainage. While it can help decrease venous stasis passively, it is not as effective as the active muscle contractions achieved through ambulation and prescribed exercises.
The nurse's clinical judgment must focus on the "most serious complication," which in the context of major joint surgery is unequivocally VTE and its progression to PE. The research consistently reinforces that prophylactic strategies, both mechanical (early mobility) and pharmacological, are the central focus of postoperative care to prevent these events [1, 3, 4].
