Understanding the Priority Assessment
The most concerning finding requiring immediate intervention is the
absence of pedal pulses with a cool, pale extremity distal to the surgical site. In the context of a post-operative total knee replacement patient, these signs are classic indicators of
acute arterial occlusion or a developing
compartment syndrome, both of which constitute a vascular emergency. The underlying mechanism involves compromised arterial blood flow to the tissues, leading to ischemia. If perfusion is not rapidly restored, irreversible tissue damage, necrosis, and potential limb loss can occur within hours. The urgency of this assessment is supported by the principle that tissue hypoperfusion is a direct precursor to adverse outcomes, including organ dysfunction and tissue death

Analysis of the Assessment Findings
A systematic post-operative neurovascular assessment is critical. The findings in option 1 represent a complete disruption of the "P" components of a neurovascular check—
Pain,
Pallor,
Pulselessness,
Paresthesia, and
Poikilothermia (coolness)—which are the hallmark signs of acute limb-threatening ischemia. This is not an expected post-surgical change.
The other options, while requiring nursing action, do not signal an immediate threat to the limb or life:

Option 2 (Moderate pain): A pain level of
6/10 is common after major joint surgery. While inadequate analgesia requires reassessment and intervention, it is an expected finding and not the highest priority.

Option 3 (Serosanguineous drainage): 50 mL of serosanguineous drainage over
8 hours is an anticipated output from a surgical drain. It does not indicate active, frank bleeding or infection at this stage.

Option 4 (Limited flexion and swelling): Limited knee flexion to
30 degrees with mild swelling is a normal post-operative expectation due to surgical trauma, inflammation, and the presence of the implant. This is the primary target of standard rehabilitation protocols, such as continuous passive motion, which are designed to progressively improve range of motion .

airway, breathing, and circulation (ABC) combined with the
acute versus chronic principle. An absent pulse and a cold, pale limb represent an acute compromise to circulation (the "C" in ABC) distal to the surgical site.
