Understanding the Priority: Infection Risk in Chemotherapy-Induced Neutropenia
For a patient with leukemia undergoing chemotherapy, all the listed interventions address genuine and serious risks. However, the question asks for the
highest priority measure to prevent a
life-threatening complication. To determine this, we must consider the most common, most rapidly fatal, and most preventable acute complication linked directly to the mechanism of chemotherapy and the disease process itself.
Chemotherapy targets rapidly dividing cells, which includes leukemia cells but also devastates the bone marrow's healthy hematopoietic stem cells. The resulting

Research on pediatric leukemia patients during induction chemotherapy-induced neutropenia (CIN) specifically identifies sepsis as a critical outcome, with studies aiming to pinpoint clinical factors that predict its onset within 24 hours of neutropenia development
[4]. The rapidity of this decline means that a patient who is afebrile and stable in the morning can develop septic shock by the evening if a small break in the skin or gut mucosal barrier allows bacteria to translocate into the bloodstream.
This is why the foundational nursing intervention is the implementation of
strict neutropenic precautions. This is not a single task but a comprehensive, evidence-based bundle of care strategies. As outlined in holistic nursing management models, effective clinical management begins with rigorous infection control practices

This includes meticulous hand hygiene, private room placement, strict avoidance of fresh fruits/vegetables and standing water that can harbor pathogens, and immediate assessment and culturing upon the first sign of a temperature spike. The goal is to create a protective barrier around the patient, compensating for their absent internal defenses. A personalized nursing pathway for infection prevention in hospitalized leukemia patients has been shown to be effective in reducing hospital-acquired infections, demonstrating the direct impact of these nursing-driven protective measures on patient outcomes .
Analyzing the Other Options
While the other options are critical components of care, they are secondary in the hierarchy of immediate life threats.

Option 1: Monitor for tumor lysis syndrome (TLS) and maintain hydration. TLS is a significant metabolic emergency caused by the rapid destruction of leukemia cells, releasing potassium, phosphorus, and nucleic acids. It is a high-priority concern, but it is a specific complication of the initial treatment phase and is managed proactively with hydration and medications like allopurinol or rasburicase. Its onset is often predictable and linked to the first 12-72 hours of treatment. In contrast, the risk of infection from neutropenia is a prolonged, persistent threat throughout the entire chemotherapy cycle, making continuous vigilance more universally critical.

Option 2: Assess for bleeding tendencies and implement bleeding precautions. Chemotherapy-induced
thrombocytopenia (low platelet count) creates a serious risk of spontaneous or traumatic hemorrhage. While a major intracranial or gastrointestinal bleed can be immediately life-threatening, the risk of a fatal infection in a severely neutropenic patient is statistically more common and can occur silently without an obvious source. Bleeding precautions are essential, but a patient can live with a low platelet count with careful protection; they cannot survive an uncontrolled gram-negative sepsis without neutrophils.

Option 3: Provide nutritional support and monitor for mucositis development. Mucositis, the painful inflammation and ulceration of the gastrointestinal tract, is a significant source of morbidity.
