200 cells/mm³, which defines severe neutropenia (ANC
< 500 cells/mm³). At this level, the immune system’s ability to mount an inflammatory response is profoundly compromised, and the risk of life-threatening infection escalates dramatically

Priority Intervention and Rationale
The highest priority is to
implement strict neutropenic precautions immediately. The rationale is grounded in the direct correlation between the depth of neutropenia and the incidence of
febrile neutropenia (FN), a major source of morbidity and mortality in this population

neutropenic nursing care (NNC) bundle is an evidence-based strategy designed to prevent infection through strict environmental controls, meticulous hand hygiene, and close monitoring for early signs of sepsis

Analysis of Other Options

Pathophysiology and Clinical Application

The body’s classic signs of infection—pus, localized swelling, and significant fever—may be absent or blunted, meaning a localized infection can rapidly progress to systemic sepsis without typical warning signs. This includes placing the patient in a private room, enforcing strict hand hygiene for all contacts, restricting visitors with any signs of illness, avoiding fresh flowers and standing water, and ensuring that all invasive lines are assessed daily for necessity and signs of infection
