When managing a mass casualty incident involving a suspected bioterrorism agent like anthrax, the initial actions are critically different from a standard medical emergency. The primary goal shifts from individual patient care to protecting the larger population, including other patients, staff, and the healthcare facility itself. The nurse's first priority is always to ensure scene and facility safety to prevent the emergency department from becoming a secondary contamination site.
Why Decontamination and Isolation Come First
The correct action is to
establish decontamination procedures and isolate potentially exposed patients. The rationale is rooted in the fundamental principles of hazardous material and bioterrorism response.
Anthrax, caused by
Bacillus anthracis, is recognized as one of the most often weaponized pathogens
[2]. In a suspected aerosolized release, as suggested by the white powder in a ventilation system, patients' clothing and skin may be contaminated with spores. If these patients are allowed to enter the main emergency department without decontamination, they risk aerosolizing the spores again, exposing an entire facility.
