The absolute priority in a chemical mass casualty incident is to establish a decontamination corridor outside the hospital. Victims must not enter the ED before decontamination to prevent off-gassing of toxic vapors.
Designate a hot zone (contaminated) and cold zone (clean) with a clear unidirectional flow. Staff in the hot zone must wear appropriate personal protective equipment (PPE) including respirators.
Never begin decontamination inside the emergency department. Volatile chemicals can incapacitate staff and contaminate critical infrastructure, turning the hospital itself into a secondary incident scene.
