Postoperative cervical hematoma: the emergency to suspect first

Neck tightness, difficulty breathing, and restlessness occurring 8 hours after a thyroidectomy are the classic presentation of a postoperative cervical hematoma. Although a cervical hematoma is an uncommon complication of thyroid surgery, it is a life-threatening emergency [1]. When bleeding from the surgical site collects in the confined space of the thyroid bed, the hematoma can directly compress the trachea and progress rapidly to airway obstruction [2].

The client's vital signs show a blood pressure of 148/90 mmHg, pulse of 108 beats/min, and respirations of 26 breaths/min, indicating tachycardia, tachypnea, and mild hypertension. These findings reflect hypoxia from airway compression with resulting sympathetic nervous system activation, compounded by pain and anxiety. Restlessness (agitation) is also an important clinical indicator of hypoxia.

Although the serum calcium of 7.2 mg/dL meets the criteria for hypocalcemia, hypocalcemia caused by parathyroid hypofunction after thyroidectomy typically develops gradually, appearing 24 to 48 hours after surgery. Hypocalcemia is therefore unlikely to be the leading cause of respiratory distress in the acute period only 8 hours after surgery. Furthermore, hypocalcemia alone does not explain the sensation of neck tightness. Securing the airway thus takes priority over interventions for hypocalcemia, such as testing for tetany or administering calcium supplements.

The nurse's priority intervention when airway compression is suspected

When a cervical hematoma is suspected, the first intervention is to assess the neck dressing and the surgical drain. The nurse must quickly check for swelling at the surgical site, the amount of blood oozing onto the dressing, and the patency and output of the drain. If the drain is kinked or occluded by a clot, a hematoma can accumulate rapidly. This direct assessment allows early identification of the cause of airway compression and prompt notification of the provider so that emergency measures, such as removing sutures and evacuating the hematoma, can be carried out without delay. This is the nurse's central role.

Airway compression from a cervical hematoma can worsen dramatically within minutes to hours. In severe cases it may lead to serious airway complications such as tracheal stenosis or a tracheal fistula, which require advanced airway management such as awake fiberoptic intubation [3]. Preparing a tracheostomy tray is therefore a reasonable follow-up measure in case the situation deteriorates, but it is not the first intervention aimed at directly resolving the cause of the current respiratory distress. Encouraging deep breathing and coughing helps clear airway secretions, but it is unlikely to be effective when the airway is being compressed externally and may instead raise intrathoracic pressure and worsen the client's discomfort.
