Nursing Clinical Practice Guide
Clinical Scenario: You are the nurse on a pediatric orthopedic unit. Your patient, a 10-year-old named Jamie, is in skeletal traction for a closed femur fracture sustained in a bicycle accident 48 hours ago. During your morning assessment, Jamie, who was previously cooperative, is now anxious, complaining of sudden, sharp chest pain that worsens with breathing. You note his respiratory rate is 32 breaths/min (tachypneic), oxygen saturation is 92% on room air (down from 99%), and you see a fine, red, non-blanching rash across his upper chest and neck.
Nursing Intervention Strategy:
1.
Immediate Assessment (ABCs): Assess airway patency. Listen to breath sounds (may hear crackles or wheezes). Check vital signs, especially oxygen saturation. Perform a focused neuro check (alertness, orientation).

Priority Action & Notification: Stay with the patient. Call the rapid response team or notify the healthcare provider
immediately using SBAR (Situation, Background, Assessment, Recommendation) communication.

Emergency Preparations: While awaiting the team, administer high-flow oxygen via non-rebreather mask as per protocol to maintain SpO2 > 95%. Ensure IV access is patent. Have emergency equipment (crash cart, intubation supplies) readily available.

Ongoing Monitoring & Support: Continuously monitor vital signs, oxygen saturation, and respiratory effort. Keep the patient in a position of comfort that facilitates breathing (often semi-Fowler's). Provide calm, reassuring communication to reduce anxiety, which can worsen dyspnea.

Collaboration & Documentation: Assist with diagnostic tests (chest X-ray, arterial blood gas (ABG)). Document everything meticulously: time of symptom onset, description of rash, vital sign trends, actions taken, and provider notifications.
Patient Safety and Precautions:
- Do NOT adjust traction weights or manipulate the limb excessively. This could dislodge more fat emboli.
- Do NOT dismiss chest pain as "muscle pain" from immobility or anxiety.
- Monitor for deterioration: FES can progress to ARDS. Watch for increasing oxygen requirements, cyanosis, and changes in mental status (restlessness, confusion).
Nursing Procedure & Medication Flow
Procedure: Responding to Suspected FES
1. Recognize the signs (Triad + fracture history).
2. Call for help (Activate rapid response/notify provider).
3. Initiate oxygen therapy.
4. Obtain vital signs and pulse oximetry.
5. Prepare for transfer to a higher level of care (e.g., ICU).
6. Assist with diagnostics (ABG, CT scan).
7. Administer medications as ordered (corticosteroids, analgesics, IV fluids).
8. Provide continuous monitoring and emotional support.
Medication Cautions: If corticosteroids are ordered, monitor for hyperglycemia and signs of infection. Analgesics for pain should be used cautiously as they can mask neurological changes or depress respirations.
