The most concerning signs are neuroglycopenic: lethargy, hypotonia, and difficulty arousing despite stimulation. These indicate the brain is starved of fuel, risking neuronal injury.
Autonomic symptoms like mild tremors or jitteriness stem from a catecholamine counter-regulatory response and are less urgent than direct CNS depression. A blood glucose of 45 mg/dL with normal feeding is typically above the operational threshold for intervention.
Do not wait for neuroglycopenic signs to develop. Screen IDMs per protocol within the first hours of life. Immediate intervention (IV dextrose or feeding) is required for any symptomatic hypoglycemia or glucose <40 mg/dL.
