Do not delay evaluation to adjust diabetic management or perform routine prenatal checks. Begin with a non-stress test (NST) to assess fetal heart rate reactivity and variability. If the NST is non-reactive, a biophysical profile (BPS) or contraction stress test (CST) should follow to comprehensively evaluate fetal status.
Instruct the client to perform daily fetal movement counts starting at 28 weeks. She should lie on her left side and count movements at the same time each day. A healthy fetus typically moves at least 10 times in 2 hours. Any report of fewer than 10 movements in 2 hours, or a significant and sustained decrease from the baseline pattern, requires immediate presentation to the labor and delivery unit for evaluation, not waiting for the next scheduled appointment.
