학습 자료 둘러보기 →The classic hemodynamic signature of left ventricular failure is an elevated PCWP (>18 mmHg), a low cardiac output (<4 L/min), and a high SVR (>1200 dynes/sec/cm⁵). The failing left ventricle causes blood to back up into the pulmonary circulation, directly increasing the PCWP.
A PCWP of 22 mmHg indicates severe pulmonary venous congestion. The priority assessment is for pulmonary edema, manifested by crackles, dyspnea, and decreasing oxygen saturation. This reflects the immediate threat to gas exchange.
Do not confuse this with a septic shock profile, which features a low SVR and normal or high cardiac output. The elevated SVR here is a compensatory vasoconstrictive response to pump failure, not hypovolemia.
