학습 자료 둘러보기 →The earliest and most reliable indicator of acute compartment syndrome is pain out of proportion to the injury that is unresponsive to analgesia. This ischemic pain precedes vascular changes.
Late signs include the 6 Ps: Pain, Pallor, Pulselessness, Paresthesia, Paralysis, and Poikilothermia. The presence of pallor and coolness indicates tissue hypoxia is already underway.
The priority intervention is immediate notification of the physician and preparation for emergency decompression, typically by bivalving or removing the cast. Intra-compartmental pressure exceeding 30-45 mmHg requires surgical fasciotomy.
Never elevate the limb above heart level or apply ice; elevation reduces arterial perfusion pressure and cold induces vasoconstriction, both worsening ischemia. Do not delay intervention to administer additional analgesia.
