Complete a full neurovascular assessment focusing on the 6 Ps: Pain, Pallor, Pulselessness, Paresthesia, Paralysis, and Poikilothermia. Cool, pale toes are a late sign of arterial compromise.
Time to intervention is critical; irreversible nerve and muscle damage can begin within 4 to 6 hours of ischemia onset. Immediate physician notification and preparation for compartment pressure measurement are the priority nursing actions.
Never elevate the affected extremity above the heart level if compartment syndrome is suspected. This decreases arterial perfusion pressure and worsens ischemia. Do not apply ice or adjust traction weights without a specific order.
