Intense pruritus, especially nocturnal, results from a type IV hypersensitivity reaction to the mite, eggs, and fecal matter. A definitive diagnosis is made by identifying the mite, eggs, or feces from a skin scraping under microscopy.
Do not confuse scabies burrows with the honey-crusted lesions of impetigo or the annular scaly patches of tinea. Crusted (Norwegian) scabies is a severe, highly contagious form seen in immunocompromised patients.
