The highest priority finding is the development of peritoneal signs, specifically increasing pain with guarding and rigidity. This signals somatic peritoneal irritation from an expanding hematoma or free blood, representing an acute surgical abdomen and ongoing, life-threatening hemorrhage.
Immediate administration of factor VIII concentrate is critical to control the bleed. Do not delay replacement therapy for diagnostic imaging. A mild headache (4/10) or joint swelling, while requiring monitoring, is a lower priority than active internal bleeding with peritoneal irritation.
