Diagnostic overshadowing—attributing new symptoms to the chronic illness—can delay treatment of life-threatening conditions like acute cholecystitis or ischemic bowel.
The priority nursing action is to prepare for immediate surgical consultation and maintain NPO status. Pain medication and IV fluids are secondary; they must not delay the surgical evaluation that will definitively rule out an acute surgical abdomen.
Never apply warm compresses to a rigid, painful abdomen. This can mask evolving peritoneal signs and delay the diagnosis of a surgical emergency. Pain management should only be initiated after the surgical evaluation is complete or per the surgeon's order.
