Hypoxemia triggers a vicious cycle: alveolar hypoxia promotes hemoglobin S polymerization, worsening vaso-occlusion in the pulmonary microvasculature. This leads to ventilation-perfusion mismatch and can rapidly progress to acute pulmonary hypertension and right heart strain.
Apply the ABC framework: a respiratory threat like hypoxemia takes immediate priority over fever, hypertension, or chronic anemia. Administer supplemental oxygen and notify the provider urgently.
