학습 자료 둘러보기 →In a sickle cell patient with chest pain, hypoxia (O2 sat < 90%) with shallow breathing is the most critical finding. This presentation is highly suspicious for Acute Chest Syndrome (ACS), a leading cause of death.
Shallow breathing, or splinting, due to pain leads to hypoventilation and atelectasis, which worsens pulmonary sickling. This creates a vicious cycle that can rapidly progress to respiratory failure and acute pulmonary hypertension.
Do not delay intervention for hypoxia to treat pain first. Incentive spirometry is critical to prevent atelectasis, and aggressive pain control is needed to facilitate deep breathing, but oxygenation is the immediate priority.
