The itching triggers a scratch-itch cycle, leading to egg collection under the fingernails and subsequent autoinfection or transmission to others via contaminated fomites like bedding and clothing.
Diagnosis is best confirmed with the "scotch tape test", performed in the early morning before bathing or defecation, to collect and microscopically identify eggs from the perianal skin.
Do not rely on routine stool ova and parasite exams, as pinworm eggs are rarely found in stool. Treat the entire household simultaneously to prevent reinfection, and emphasize strict hand hygiene.
