학습 자료 둘러보기 →The classic presentation in a 3-year-old includes steatorrhea (bulky, foul-smelling, fatty stools), abdominal distention, and failure to thrive despite adequate intake. These result from villous atrophy and subsequent malabsorption of fats, proteins, and micronutrients.
Abdominal distention is caused by the fermentation of unabsorbed carbohydrates by intestinal bacteria and hypoproteinemia-induced edema in the gut wall.
Do not confuse the steatorrhea of celiac disease with bloody, mucoid stools from infectious colitis or the projectile vomiting of pyloric stenosis. A detailed dietary history focusing on gluten intake is critical for assessment.
