학습 자료 둘러보기 →In a client with dysuria and frequency, the most critical assessment is for signs of upper UTI (pyelonephritis). The combination of fever (>100.4°F / 38°C) and costovertebral angle (flank) tenderness indicates systemic infection and renal parenchymal invasion, requiring immediate provider notification.
Uncomplicated cystitis typically presents with local symptoms like cloudy, odorous urine and positive dipstick findings (nitrites, leukocyte esterase). While diagnostic, these do not signal the life-threatening progression that fever and flank pain do.
Never dismiss fever and flank pain as a simple UTI. This presentation carries a high risk for bacteremia and sepsis. Anticipate orders for blood cultures, parenteral antibiotics, and imaging to rule out obstruction or abscess.
