학습 자료 둘러보기 →In stage 4 CKD, a serum potassium of 6.8 mEq/L with peaked T waves is a medical emergency. The loss of renal potassium excretion leads to rapid accumulation, directly destabilizing cardiac myocyte membranes and risking progression to ventricular fibrillation or asystole.
Immediate nursing actions include placing the patient on a cardiac monitor, obtaining a 12-lead ECG, and notifying the provider. Anticipate emergent orders for IV calcium gluconate to stabilize the myocardium, followed by therapies like IV insulin with dextrose to shift potassium intracellularly.
Do not delay treatment for repeat lab confirmation if ECG changes are present. A potassium level above 6.5 mEq/L with ECG changes indicates immediate danger of cardiac arrest, and calcium is the priority to protect the heart.
