학습 자료 둘러보기 →The nurse's first action is always a focused respiratory assessment including SpO2, respiratory rate, depth, and effort. This establishes the baseline needed to safely titrate oxygen and detect an acute exacerbation.
In COPD, the respiratory drive may shift from a high-CO2 stimulus to a hypoxic drive. Uncontrolled oxygen can blunt this drive, leading to hypoventilation, rising PaCO2, and respiratory acidosis.
The goal is not a normal SpO2 of 95-100%, but a prescribed target range, often 88-92%. Titrate oxygen to the lowest flow rate that maintains this target saturation.
Never apply a standard high-flow rate like 6 L/min to a COPD patient without a prior assessment and a specific order targeting a saturation range. Doing so risks inducing life-threatening hypercapnic respiratory failure.
