Check the 6 Ps (Pain, Pallor, Pulselessness, Paresthesia, Paralysis, Poikilothermia) every 15 minutes to detect compression of the profunda femoris artery or nerves by bone fragments.
Maintain the patient in a supine position with the leg in neutral alignment. Avoid hip flexion by not elevating the head of the bed more than 30 degrees. This minimizes the risk of further fragment displacement and vascular kinking.
A sudden change in pain quality (deep, unrelenting), loss of palpable pedal pulses, or new-onset paresthesia is a surgical emergency indicating acute compartment syndrome or vascular injury. Notify the surgeon immediately and document findings precisely.
