Lesions predominantly appear on areas with the highest density of sebaceous glands: the face, chest, and back. The upper arms and shoulders may also be involved.
A comedone is the primary lesion. An open comedone (blackhead) appears as a dilated follicle with a dark center, while a closed comedone (whitehead) is a small, flesh-colored papule without an obvious opening.
Do not confuse acne pustules with the clear, fluid-filled vesicles of viral infections like herpes simplex or the wheals of urticaria. Acne lesions are centered on follicles and do not rapidly appear and disappear.
