학습 자료 둘러보기 →In pulmonary-renal syndromes like Goodpasture's, the immediate threat to life is diffuse alveolar hemorrhage (DAH). Blood in the alveoli blocks gas exchange, causing rapid hypoxemia and potential acute respiratory failure. The nursing priority is to secure the airway and optimize oxygenation.
Continuously monitor oxygen saturation and assess for signs of respiratory distress (tachypnea, use of accessory muscles). Be prepared to administer high-flow oxygen and support the patient in a high-Fowler's or upright position to maximize lung expansion. Suction equipment must be at the bedside to clear the airway of blood if needed.
Do not place the patient in a Trendelenburg position. This would worsen respiratory mechanics and increase the risk of aspiration. While hematuria indicates renal involvement, the ABC framework dictates that airway and breathing complications are managed before renal concerns.
