It should be ordered when a patient presents with signs of systemic involvement like arthralgias, unexplained fever, or malar rash. The result is reported as a titer (e.g., 1:80) and a pattern.
A positive ANA is not diagnostic on its own. Up to 15% of the healthy population can have a low-positive ANA. Diagnosis requires correlating the lab value with specific clinical criteria and follow-up tests like anti-dsDNA or anti-Smith antibodies.
Do not use a negative ANA to rule out all autoimmune diseases; it is highly sensitive for SLE but may be negative in conditions like seronegative spondyloarthropathies. Always correlate with the patient's symptoms and physical exam.
