While radiation pneumonitis typically presents with a non-productive cough and low-grade fever, a productive cough suggests an infectious etiology.
Assess vital signs and oxygen saturation immediately. Anticipate orders for a chest X-ray, blood cultures, and sputum culture to identify the source. Prompt initiation of antibiotics is critical if infection is suspected.
Do not assume all respiratory symptoms are radiation pneumonitis. A temperature spike above 100.4°F (38°C) with purulent sputum is a red flag for a bacterial process that can rapidly progress in an immunocompromised host.
